Emily Cashman, RN, FNP

Emily Atwood

Emily Cashman  graduated with a BA in Biology from the University of Maine at Farmington in 2003. She received her BSN in Nursing from the University of Southern Maine College of Nursing in 2005 and her MS in Nursing and FNP through also through the University of Southern Maine College of Nursing in 2007. She is a Life Member of the Phi Kappa Honor Society and is a Member of the Sigma Theta Tau Nursing Honor Society. Her clinical experience before she joined FHCA included Maine Medical Center's Brighton First Care Clinic and Mercy Express Care, Coastal Health Care Family Practice, Village Pediatrics in Waterboro and the Maine Medical Center Outpatient Care Service. She joined FHCA in 2007.




Amy Bergeron, RN, FNP

Amy Bergeron

Amy Bergeron received her BSN in Nursing from St. Joseph's College in 1998 and worked as a nurse and charge nurse in the St. Mary's ICU. She graduated from the SUNY at Stony Brook nurse practitioner program in 2002. She is certified as a Family Nurse Practitioner and a member of the American Academy of Nurse Practitioners. She is a runner and avid NHL and Bruins fan. She joined us in 2002.




Charles Burns, MD

Charles Burns

Charles Burns earned his BA degree at the University of California, Santa Cruz, and graduated from the University of California at San Francisco School of Medicine. He completed two years of post- graduate training in Social Medicine at Montefiore Hospital, Bronx, New York and two years at the Family Practice Residency at Central Maine Medical Center, Lewiston, Maine. He is Board Certified in Family Practice. After completing his residency, Dr. Burns spent two years in private practice in Oakland, California before returning to Lewiston as a faculty member at the CMMC Family Practice Residency, where he was Associate Director for two years. He joined Family Health Care Associates in 1990. Dr. Burns enjoys sailing on Maine's lakes and along the coast.




Suzanne D'Angelo, MS, RD, LD

Susan D'Angelo

Suzanne D'Angelo is a registered dietician. She earned her Bachelor of Science in Food and Nutrition in 1989 and her Master of Science from the University of Maine in Food Studies and Human Nutrition in 1999. She has been working at Saint Mary's Medical Center. She joined us in 2002.




Peter Elias, MD

Peter Elias

Peter Elias earned his BA degree at Dartmouth College and graduated from the University of Rochester School of Medicine. During medical school he completed a fellowship in Clinical Pharmacology. He completed three years of post-graduate training in Family Practice at the University of Wisconsin at Madison and is Board Certified in Family Practice. After his residency, he worked for a year as the Ambulatory Care Director and an Emergency Room Physician at Central Maine Medical Center. He has served as Chief of the Department of Family Practice at CMMC and is Chair of the CMMC Bylaws Committee. He has received additional training in hypnosis and hypnotherapy, and has a special interest in sports medicine and exercise physiology. Leisure activities include soccer, nordic skiing, hiking, genealogy, computers and guitar. Dr. Elias was one of the original two founders of Family Health Care Associates in 1977.




Tom Meek, DO

Tom Atwood

Dr Meek received his certification as a Registered EMT from Hocking Technical College, Ohio, in 1994 and worked as an EMT through 1998. He graduated cum Laude with a BS in Biology from the University of Rio Grande, Ohio, in 2002 and trained in combined Family Practice and NeuroMusculoskeletal Medicine at the University of New England and Southern Maine Medical Center receiving his DO in 2006. In addition to Family Medicine, Dr. Meek has special interests in pain management, addiction medicine and international medicine. He has participated in Medical Mission experiences in Peru, Honduras and Kenya. He joined FHCA in 2008.




Dianne Paine, RN, FNP

Dianne Paine

Dianne Paine is a graduate of the University of Southern Maine with undergraduate degrees in Business Administration and Nursing. She also has a Masters of Science in Nursing. She is certified as both a Family Nurse Practitioner and a Psychiatric and Mental Health Nurse. She joined us in the summer of 2000.




Michael Rifkin, MD

Mike Rifkin

Michael Rifkin earned his BA degree at the State University of New York at Buffalo and graduated from Boston University School of Medicine. He completed three years of post-graduate training in Family Practice at the Residency Program in Augusta, Maine and is Board Certified in Family Practice. After completing his residency he practiced as a family physician for two years at the D.F.D Russel Medical Center in Leeds. He has received additional training in osteoporosis. He enjoys hiking, gardening, biking and skiing. Dr. Rifkin joined Family Health Care Associates in 1982.




Sara Skelton, MD

Sarah Skelton

Sara Skelton joined us in August of 2001. She graduated with a B.A. from the University of Chicago and then completed a baccalaureate premedical program at Bryn Mawr College in Pennsylvania. She graduated from Dartmouth Medical School and completed a three year post-graduate Residency Program in Family Practice at the Central Maine Medical Center in Lewiston, Maine. She was Chief Resident during her last year of residency. She enjoys the out of doors and horseback riding.




Raymond Tardif, MD

Ray Tardif

Ray Tardif was born and raised in Auburn and attended Edward Little High School. He earned his BA degree at the University of Utah and graduated from the University of Utah School of Medicine. He completed three years of post-graduate training in Family Practice at the Residency Program at Central Maine Medical Center in Lewiston, Maine where he was Chief Resident during his last year. He is Board Certified in Family Practice. His leisure activities include telemark skiing, canoeing and kayaking, squash and tennis. He joined Family Health Care Associates in 1998.




Jeri Wade, RN, FNP

Jeri Wade

Jeri Wade completed her nursing training at the Central Maine Medical Center School of Nursing. After working six years as an Emergency Room Nurse, she completed the Family Nurse Practitioner Program at the University of Maine in Portland and worked for several years in the Emergency Department of Central Maine Medical Center. She has a strong interest in adolescent medicine and well woman care and has been with Family Health Associates since 1986.
